WIC Appointments

WIC is part of the Clermont County General Health District Nursing Division. We are located at 2400 Clermont Center Drive, Suite 204. We also schedule WIC appointments at the Health District’s Immunization Clinics.  WIC is open Monday through Friday 8:30 a.m. - 4:30 p.m.. Please call to schedule an appointment. We offer some late appointments on Mondays.

What to bring to your certification or recertification appointment:

• You may print and fill out a WIC Application and the Infant Health History Form or the Child Health History Form, Pregnant Women Health History Form, and the Breastfeeding and Postpartum Health History Form before your appointment.
• Child, bring everyone with an appointment
• Identification for mother which may include WIC ID folder, driver's license, social security card or medical card
• Identification for child which may include WIC ID folder, medical card, shot records or birth certificate
• Proof of income evidenced by the last 30 days pay stubs plus any other household income or current proof of public assistance i.e. Medicaid (medical card), food stamps, OWF, or disability assistance if applicable.
• Shot records
• Proof of pregnancy

What to bring to your nutrition education (coupon pick-up) appointment:

• Identification for mother which may include WIC ID folder, driver's license, social security card or medical card
• Identification for child which may include WIC ID folder, shot records, medical card or birth certificate
• Childs shot record.
