WIC is part of the Clermont County General Health District Nursing Division. We are located at 2400 Clermont Center Drive, Suite 204. We also schedule WIC appointments at the Health District’s Immunization Clinics. WIC is open Monday through Friday 8:30 a.m. - 4:30 p.m.. Please call to schedule an appointment. We offer some late appointments Monday through Wednesday.
What to bring to your certification or recertification appointment:
Child, bring everyone with an appointment
Identification for mother which may include WIC ID folder, driver's license, social security card or medical card
Identification for child which may include WIC ID folder, medical card, shot records or birth certificate
Proof of income evidenced by the last 30 days pay stubs plus any other household income or current proof of public assistance i.e. Medicaid (medical card), food stamps, OWF, or disability assistance if applicable.
